Citizens of the Commonwealth engaged in multi-level collaborations with anti-apartheid groups, while state government supported divestment legislation, and institutions of higher education provided support for academic scholarships and other training efforts. Recognizing the important role played by many in Massachusetts, Mr. Nelson Mandela included Boston as one of the stops he made on his first US visit in 1990. In 1994, Massachusetts began taking steps towards the formalization of support for the newly elected government of South Africa, and a year later, Governor William Weld led a trade mission to South Africa. In 1997 he signed a Sister State/Province Agreement between the Province of the Eastern Cape and the Commonwealth of Massachusetts that prioritized the development of academic relationships, economic development programs, private investment and community partnerships. Following the Governor s lead, Dr. Howard K. Koh, then-Commissioner of the Massachusetts Department of Public Health (MDPH), took steps to develop a bi-lateral relationship in the area of public health, and in 1998 signed a letter of cooperation with the Eastern Cape Department of Health (ECDOH). As part of this effort, the Massachusetts  South Africa Health Task Force was established in April 2000 to support government-to-government activities and mutually beneficial collaborations. During its first two years of operation, the Task Force has outlined three broad areas in which potential collaborations are considered. Support for the development of the provincial Department of Health's capacity to survey, plan for, resource, and manage the delivery of public health and medical services in the province; Support for and projects with Eastern Cape institutes of higher education in their evolving collaborative efforts with the region's district and local health departments and the NGO sector. Assistance to selective non-governmental organizations (NGO) whose goals and activities are of mutual public health interest to the Province and to collaborators here in Massachusetts. In order to carry out its program, the Task Force has established three groups - the Academic Working Group, NGO Working Group, and the Departmental Working Group  which develop and support program activities. The Task Force is also committed to supporting the activities of industry and trade partners seeking to improve health care in South Africa. Selected accomplishments from the Working Groups include: Academic Working Group: A video teleconference with the Massachusetts academic representatives and University of Transkei (Unitra) faculty that identified preliminary priority areas including student and faculty exchanges, joint research, and the further development of telemedicine and informatics capacity. University of Massachusetts Medical Center  Unitra joint research lab over the course of four years that involved students from both institutions. NE Medical Center, Department of Telemedicine hosted a Unitra professor for a 3- month training course. With the help of working group members, a Unitra faculty member was selected as an international fellow by the Foundation for Advancement of International Medical Education and Research in Pennsylvania. Massachusetts Medical Society hosted two public meetings, one to explore issues of sexual assault with representatives of the Masimanyane Women s Support Centre from South Africa in March 2002, and a second to take place in April 2004 featuring the director of the Foundation for Professional Development (a subsidiary of the South African Medical Association) to explore training opportunities for medical personnel on issues related to HIV/AIDS case management. Massachusetts General Hospital Institute for Health Professions (MGH/IHP), working with the Unitra Faculty of Nursing, ran a weeklong workshop on HIV/AIDS Case Management for nursing personnel from surrounding health facilities. NGO Working Group: Codman Square Health Center and Masimanyane Women s Support Centre signed the first formal NGO twinning agreement on March 8, 2002. Two Eastern Cape Health NGO Surveys were conducted in 2001 and 2003, resulting in a Directory of health-related NGOs in the Eastern Cape. Cambridge Health Alliance (a complex of three hospitals affiliated with Harvard Medical School) and the Port Elizabeth Complex (three formerly race-based hospitals that have merged into one complex) have established joint working groups on issues of hospital management, human resource development, information technology, and clinical support. A Cambridge Health Alliance doctor traveled to Unitra for 5 weeks to conduct a baseline study of Volunteer Counseling and Training (VCT) programs in the area. Codman Square Health Center and Masimanyane Women s Support Centre received a US State Department grant to establish a Masimanyane Technology Centre in South Africa that is linked with the Codman Technology Center in Boston, and to develop a joint training curriculum that draws the connection between HIV/AIDS and domestic violence. Departmental Working Group: Four visits by ECDOH officials to Massachusetts, and five visits by MDPH officials to the Eastern Cape since 1999. Hosted a Dean s Breakfast meeting attended by 20 representatives from 17 schools of medicine, public health, nursing, pharmacy, and dentistry in Massachusetts. Follow-up meetings have resulted in several new partnerships. MDPH AIDS Bureau provided technical support to the University of Transkei for its proposal submission to the World AIDS Foundation, resulting in a grant to test a new curriculum for rural community health workers. MDPH AIDS Bureau provided technical support to the ECDOH HIV/AIDS Directorate for its submission to the CDC for funds to establish a Regional Training Centre for HIV/AIDS in the Eastern Cape. MDPH AIDS Bureau consulted with the ECDOH HIV/AIDS Directorate on management of ARV treatment plan implementation. Planning for a major people living with HIV/AIDS (PLWHA) conference in the Eastern Cape for 2004, based on the Massachusetts Consumer Advisory Board Conference held each year to disseminate the latest information about HIV/AIDS to Massachusetts consumers and practitioners. The Task Force and MDPH have created a unique relationship that is the only one of its kind in the United States.
